Will the large size fit like a 5~6 inch hermann's tortoise??

The L harness is adjustable. Are we talking width or length in your tortoise sizes? I've attached a vodeo I hope that helps?

What does it com with

It comes with a harness and a small leash the harness is leather-like so it does have some grip on the turtle shell and the leash has a cute little bell and has a small metal chain and lether at the hand loop. It even comes with instructions, it's great!

Why can't this be used in a river?

Because when turtle in river it become stronger of the power which can struggled off from the chest collar.

Do you carry a small size? the medium was huge for my eastern box turtle

I bought it for my Russian tortoise which is about the size of my palm.
