How much are you supposed to use per application?? Each syringe is labeled 10 ml, but directions say use 1/4- 1/5 cc.?

It would take at least 3-4 days, depending on how much you apply to each tooth...it took me 5 days to get at least a bit over half way with the tube.

Should I refrigerate these?

I do. Advice on Google recommends doing so.

How long do you keep trays in mouth?

Start with 10 minutes if u have no sensitivity to ur gums then go to 20 minutes. I don't use everyday every other day is ok for me.
