Is the zipper plastic and does it have a pull tab? Where is it located?

The zipper is not plastic. I'm not sure what you mean by pull tab? The zipper part is located on the side. You put the blanket in from the side, which works for me because my daughters bed is against the wall but it also has a "lip" to cover the zipper part so it can be hidden if your bed is not against a wall... it's a really nice duvet set...

Does this have corner ties to hold the insert in place?

Don't remember because we sent it back. Our problem was it wea an odd shape and did not come close to fitting our Cal. King Duvet. Very disappointed!

Does this insert have corner ties so it will stay in place?

I believe that this one did have corner ties, however I have already gotten rid of this duvet. If you plan on washing often the zipper is weak and broke after 4 removals and washes. But super soft.

Does this duvet come with pillow cases?

It comes with shams but not pillowcases. Beautiful and high quality set.
