whats the difference between size 4t and 4 ?

The 4t allows for toddlers. Usually a little wider and shorter. Regular 4 is longer and a bit narrower, as diapers aren't usually used at this stage.

Are these Soft overall? Is the waistband Soft?

Yes, the fabric is soft. The waistband has the elastic feel but it does not seem to bother my son. He ABSOLUTELY loves them!

Is it more padded in the middle?

Not anymore than any other boys underwear.
