Can you pack just one bib for on the go or do you need to put the whole pack in the diaper bag?

Yes you can. You can add as many bibs to your diaper bag as you please. Their all in a box but single bibs

What size is the opening?

The opening is big enough to use on toddlers. I have a 2 &1/2 year old that I use them on and I've also used them on my 31/2 year old also. The ipening is big enough that you can make them as big or as small as you need them.

Has anyone tried this in the car for a toddler who gets car sick ?

I've never used it for car sickness but it sure holds drink that my toddler spit out and let drool down in the bib. It held almost a sippy cup of juice.
