Will this test the genotype or just phenotype? Meaning that is this test will test for genetic disease that my dog would pass on? Most tests don’t

It just shows what medical issues most common to dogs and your breed. And what risk level your dog is at for those problems. I didn't see anything about how likely they are to pass anything along. If you're looking for a test for breeding purposes I would ask your vet, just because this test gives very basic info.

Will this test confirm a dogs age?

Hello! The Geno Pet + kit will not confirm age.

What is the website? My kit had no info other than the swab and an envelope to mail it in. It asked for my email, but no other info.

Hello! The website is listed on the inner flap of this kit! orivet.com/activate

The website is hard to figure out. Where do you find the adult weight prediction?

Hello! When logged into your account, on your pet's profile, click on the Resources button and then click on the Nutrition button.
