If you’re using for spica cast I suggest you use pantiliners. Easy to remove and replace for a leakages

Can this be used directly on inside of shoes?

Yes. I used it in my shoes at the heel when I had a blister. I also used it during pandemic on my face mask that I had to wear 8 hrs at my job. I cut it small and then let the string be in the center and enclosed it, Worked well.
