I'd like to order the disposable clothing protector that attaches to the top of your shirt. Is this quanity of one include the 50 sheets?

Each of the 50 "bibs" is a complete unit. There are 50 bibs to a box.

Where are these made?

I purchased these once and decided to go with a different product although they were ok.

can one or two be folded up and put in one's pocket without the adhesive being compromised

The adhesive has a paper strip protecting it..guess you'd have to fold it carefully to ensure that the strip stays in place.
